Appointment as an Agent/Producer

To become appointed with SFIC, you will need to provide the following:

• Agent Appointment Packet (SFIC Producers Agreement, SFIC Limited Appointment Application, W-9 Form)
• Current Errors and Omissions Declarations Page
• Current Agency and Individual Licenses for states where your agency will place business

Submission Requirements

Submit applications via:

• Online application for Fitness Professionals
• PDF versions of all application forms

Additional information that may be needed:

• Membership/client/student contract
• Waiver/Hold Harmless Agreement
• Loss history for last 3 years
• Resume of owner for new venture
• Martial Arts sparring rules

Payment and Invoicing

Agency Bill -- with very few exceptions, we will bill you, the agent or broker, for premiums due.

• Financing options: 25% down with house finance company or outside financing
• You can withhold your commission when making payment

Marketing Support

We offer customized solutions for franchisers or associations, and development assistance for fitness business books.
